Community Grant Report
Organizations who receive funding will need to complete the following report on the manner in which the funds were spent. This report is due within one year of the grant being issued and must be received before a new application may be submitted and considered. (Please refer to page 2 of your copy of the Grant Agreement for the due date.)

You will also need the date the grant was issued to enter below (date the Grant Agreement was signed by Sherri Wise with OFF). If you need a copy of the signed Grant Agreement, please reach out to Michele at MCaine@OsteopathicFounders.org.
