Participation Fee $40.00
CONSENT TO CHARGE CREDIT CARD | As the parent or legal guardian of the above-named participant, I hereby give my consent to charge my credit card currently on file in my parent portal (Jackrabbit) I understand this is a one-time fee of $40.00 (single student/or $50.00 family maximum) non-refundable. If student is later unable to participate, parent/guardian acknowledges this fee will not be returned.
