Our approach to Wellbeing is strongly informed by the Te Whare Tapa Wha model with its dimensions of hauora/wellbeing. The following questionnaire asks how you feel about your quality of life, health, or other areas. Please answer all of the questions, if you are unsure about a response please choose the one that appears most appropriate. This can often be your first response. We ask that you think about your life in the last two weeks.
