Aerial Enchantment Student Waiver Form

I acknowledge and accept the safety protocols implemented by Aerial Enchantment in conducting their aerial dance classes and workshops. I understand that I am solely responsible for any incurred medical costs resulting from my actions during these sessions. In the event of needing medical assistance, I authorise individuals authorised by Aerial Enchantment to seek appropriate medical aid on my behalf. I agree to indemnify and hold harmless Aerial Enchantment, its owners, staff members, and guest artists from all reasonable costs, charges, liabilities, or payments stemming from any circumstances, including damage, loss, injury, or death during my participation. I acknowledge my physical limitations and agree not to exceed them during classes.

If I am a parent/guardian of a child under 18 participating in these classes/workshops, I affirm that I agree to these conditions on their behalf.

All fees for classes must be paid in full prior to the start of the class. Notification of inability to attend 24 hours before the class allows rescheduling for a makeup class.

If you have booked in for a term class and are no longer able to attend you will be given the option to rebook for a different class or transfer the class to another participant.

Refunds will not be issued for any reason, including change of mind, illness, or inability to attend.
