Need to Verify an Employee's Credential?

If you an an employer or agency and wish to verify a practitioner's certification, please complete the following form. Email notifications will be sent to the requestor's email address provided below. Note: Only one (1) employee name verification per request, multiple credentials / certificate information may be applicable.
There is a $25 fee for verification. We will confirm the credential requested. [Free for Active Military request]

We do NOT verify the following: OTC, Surgical Assist, Nursing, Occupational or Physical Therapy.
