To keep our programs high-quality and free, students must agree to a few important expectations. Please review the information carefully. Castro Valley Adult and Career Education is proud of the many accomplishments of our students and staff, and we want to increase public awareness and promote the continuation and improvement of our education programs. We annually request permission to use your photograph, name, audio recording, and video image in instructional or promotional materials and publicity efforts. This permission would allow us to use your name and image in brochures, print ads, the school website, social media, or other forms of promotion. The adult school may share student photos/videos with legislators, school district personnel, and community leaders to communicate the goals and achievements of our adult school students. Additionally, I agree that I shall have no right, title, or interest in these published images or recordings, nor shall I find cause for action or injuries reported from them. I understand that I will not receive monetary compensation for any authorized publication.

 Does Castro Valley Adult and Career Education have your permission to use your photo, name, and audio/video in our instructional and promotional materials without compensation or rights to these materials? If you choose 'no,' you will not be included in program pictures, newsletters, and yearbooks.

All Castro Valley Unified School District (CVUSD) students and staff have the right to participate fully in the educational process, free from discrimination and harassment as stated in the California Education Code 201, in the Castro Valley Unified School District Board Policy 5145.3 Hate-Motivated Behavior, and in Student Handbooks.

Over the past few years we have seen an increase in the usage of harmful and hateful language in our schools. The purpose of this compact is to eliminate all forms of racially offensive language, specifically anti-Black racist language, as well as other discriminatory language and conduct in CVUSD.

These forms of harassment create unsafe and hostile environments that are NOT aligned with the district’s mission, vision, and core values that promote welcoming, safe and inclusive educational spaces. Unsafe/Hostile environments where the use of hateful language, either verbally, in writing or through images and social media, regardless of intention, negatively affect a student’s right to a healthy, safe learning environment and education. CVUSD will not allow discrimination and harassment regardless of race, religion, gender identity or orientation.

CVUSD must be a place that is safe, welcoming, and inclusive, where all students feel a sense of belonging.

This compact will be communicated in a variety of ways including through advisory lessons, school-wide communication, districtwide communication, caregiver communication etc. and will continue to be reiterated throughout the school year.

As a student in Castro Valley Unified School District, I commit to contributing to a place of belonging for ALL students and staff by helping to create a welcoming, inclusive, and safe environment. I agree to not use racially harmful language (including use of the n-word) nor engage in other forms of hate language and acts, regardless of intent. I understand that any actions that I engage in that violate this compact will result in immediate consequences that could include loss of school activities up to and including school suspension. My actions and attitudes will reflect my commitment to help create a community where everyone feels valued, respected, and safe

I authorize releasing my education, training, employment, and other related information to Castro Valley Adult and Career Education to facilitate the services to which I may be referred. All agency and associated partners and their staff are bound by contract and by law to maintain my confidential information according to the standards set forth by the Federal Education Rights and Privacy Act (“FERPA”), by the California Education Code section 49073.1 and as appropriate, by the Health Information Privacy Protection Act (“HIPAA”).

FERPA allows schools to disclose those records, without consent, to the following parties or under the following conditions (34 CFR § 99.31):

• School officials with legitimate educational interest;
• Other schools to which a student is transferring;
• Specified officials for audit or evaluation purposes;
• Appropriate parties in connection with financial aid to a student;
• Organizations conducting certain studies for or on behalf of the school;
• Accrediting organizations; o To comply with a judicial order or lawfully issued subpoena;
• Appropriate officials in cases of health and safety emergencies; and
• State and local authorities, according to specific State law

This consent may be canceled at any time. Subject to cancellation, this consent form for data sharing will remain in effect for three years.
