BACKGROUND CHECK POLICY

Please do not apply for membership to NexJen BJJ if you are not willing to complete a background check as part of enrollment.

While running background checks on our members may seem excessive, our purpose is twofold; first, we are committed to providing a safe environment for the children who train here and peace of mind for their parents. Secondly, we want to ensure that persons who have violent felony offenses* in their history are not receiving training that could be used to harm others.

NexJen BJJ WILL NOT refund enrollment fees paid if the potential member refuses to complete a background check or an adverse report that would preclude membership is returned.

*felonies, offenses involving minors, and/or assault, battery, etc

NOTE An applicant with a conviction for misdemeanor charges will not necessarily be disqualified from membership; please speak with Coach Tabscott PRIOR to beginning the enrollment process.

TRAINING PASS AGREEMENT
- A training pass is a pre-paid membership to NexJen BJJ and Martial Arts and includes access to all regularly scheduled classes and open mats. Special events, add on classes and seminars are NOT included.

-Training passes may be renewed. Renewals made within 14 days of the pass end date do not require an additional enrollment fee.

-The duration of your training pass is calculated from the 1st day of the month closest to the date of purchase. Mid-month purchases of training passes may be subject to a prorated fee at the discretion of NexJen BJJ.

-Training pass membership may not be paused without the written consent of NexJen BJJ. Pauses of membership may be granted at the discretion of NexJen BJJ for reasons such as a prolonged injury or illness that prohibts class attendance.
Submission of this form and payment of your invoice is your acknowledgement of your acceptance of the conditions of the training pass agreement*

Yes, I have reviewed the training pass agreement. I understand that payments for a training pass are non-refundable. I understand that final membership/training access is contingent upon my completed background check and that failure to complete a check or an adverse report will result in termination of the enrollment, in which case, all fees paid are non-refundable.
