    Submitting this form is non-binding. I will calculate your fee based on your selections, then send you a fee summary for your approval. My fees are based on the first two parts below, the third is optional. 1. The type of appointment and the day you choose, $395, $495 or $650 etc. 2. Travel to your home. From $85 3. Transfer of your pet to a cremation facility if needed by me. (Optional) $195 or $150
