Details of person completing form. Completing this form will usually facilitate an initial visit to the day centre during which the prospective client can see if the day centre might be for them and during which it can be assessed if they have a similar profile to existing clients in terms of individual initiative to embark on and settle on activities and has a similar level of intellect to existing clients (though those who don't may still potentially join if they're accompanied by their own one to one support worker).

This initial visit can then be followed by three, free assessment sessions after which there will be discussion regarding a possible day support contract. There may be a potential brake applied after the first assessment session if the prospective client is not content or if it is clear that their initiative to embark on and settle on activities does not meet a baseline expectation.
