For kids with cancer ages 5-17 and their immediate family members.
Welcome to Happiness Is Camping's application! This application form is for Oncology Family Camp. Family Camp is for kids who have or had cancer, aged 5-17, and their immediate family members. Family Camp dates are as follows:
- Memorial Day Weekend, Saturday, May 24 – Monday, May 26
- Friday, June 6 - Sunday, June 8
- Friday, June 20 - Sunday, June 22
*Please note that families who cannot come for a full weekend may also register to attend for a day.

Eligibility Requirements

Please note that only immediate family members are eligible to attend. This includes parents, legal guardians, and siblings.

Summer Camp Registration
Summer Camp is for children who have or had cancer and their siblings, ages 6-17 years old. Campers stay overnight in air conditioned cabins with two staff members and up to eight campers. Pediatric oncologists and nurses are onsite to provide 24/7, comprehensive medical care. Summer Camp features many of our favorite traditions, including Prom, Talent Show, Color Wars, and more!

Summer Camp Dates:
- Session 1: Sunday, July 6 - Friday, July 11
- Session 2: Sunday, July 13 - Friday, July 18
- Session 3: Sunday, July 20 - Friday, July 25
Summer Camp is completely free to attend. Siblings MAY attend without the child with cancer.

Paperwork Requirements
Mandatory paperwork must be submitted by May 1. Please be advised that registration is not 100% complete until all mandatory paperwork has been uploaded. Camp Director Ryan Campbell will email you with instructions to upload required paperwork.

Paperwork Requirements to Attend

The following items are required to attend. Paperwork requirements are the same for families visiting for a day:
- Health Form for the child with cancer
- Immunization Record for the child with cancer
- Family Camp History Form
Additionally, all families must bring proof of health insurance for each attendee with them to camp.
If you have them readily accessible, you can upload any of these files here. Please note that this is not required at this time.
