I acknowledge that I must adhere to the policies. I understand that rescheduling or cancellations must be done with at least 48 hours notice  Failure to do so will result in the loss of my deposit or 50% of the total service cost. I acknowledge that ANY no show will result in the loss of appointment or $25 cancellation fee. I understand that after 15 minutes of tardiness my appointment may be subject to cancellation and deposit will be forfeited.

 

Client Concent 

          I acknowledge that my skin might experience temporary irritation, tightness, redness or slight swelling which usually dissipates within 72 hours depending on skin sensitivity. 

     I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions. I acknowledge that I should avoid the use of Retin-A type products, aggressive exfoliation, waxing, and products containing acids that are no part of the recommended take-home regimen for 2-4 weeks following treatment.

         I acknowledge that if I fail to use a minimal sunscreen (SPF45), I am more susceptible to sunburn, skin damage & hyperpigmentation. I should avoid excessive sun exposure especially between 10am-2pm.

        I understand how important it is to follow all instructions given to me for the post treatment care, In the event that I may have additional questions or concerns regarding my treatment or suggested home care, I will consult the Esthetician immediately.

      I consent (to the best of my knowledge) Given an accurate account of my medical history, including all known allergies or prescription drugs, or products I'm currently ingesting or using topically.

     I acknowledge that this treatment is strictly elective cosmetic procedure & no medical claims have been expressed or implied. The result of the procedure can be affected by the following medication, skin characteristics (Dry, oily, sun-damaged, thick or thin skin type),personal pH balance, skin, alcohol intake, and smoking post procedure. I understand the procedure and accept all risk. All of my questions have been answered to my satisfaction and I consent to the terms of this agreement. I do not hold the Esthetician responsible for any of my conditions that were present, but not disclosed at the time of this treatment, which may be affected by the treatment performed today.

     I release Aceluxury.skin and its staff of any liability associated with any injuries and /or current and future conditions resulting from the skincare procedures or products. I give consent to all future treatments.

 
Photo & Video

       I hear by grant permission to the right of my image, likeliness and sound of my voice on audio, photographs and videotape without payment or any other consideration. I understand that my emerge may be edited, copied, exhibited, publish or distributed, and waive the right to inspect or approve the finish product, where in my likeness appears. I understand that the images or videos may be used in various media formats, including, but not limited to print material, website, social media platforms, and online advertisement. 

        Additionally I waive any right to royalties or any other conversation or rising or relative to the use of my image or recording. I also understand that the material may be used in diverse educational setting within an unrestricted graphic area. photographic video may be used for the following purposes ( Conference, presentations, educational, presentations, or courses, informational presentations, educational videos and advertisement.

         By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via Internet or in the public educational setting. I will be consulted about the use of the photographs or video recordings for all purposes other than those listed above there is no time limit on the release nor is there any geographic limitation on where these materials may be distributed.

           I acknowledge that while Aceluxury.skin for take responsible measurement to maintain the confidentiality and privacy of my personal information, I understand that once images or videos are made public, they may be viewed accessed by others.

        By signing this form, my acknowledge that I have completely read and fully understand the above release and agreed to be bound thereby. I hear by release any in all claims against any person or organization utilizing this material for educational purposes.
