Consent and Release Form for ABF Hoops Initiative Participation
By signing below, I acknowledge that I am voluntarily enrolling myself/my child in the ABF Hoops Initiative, a program of The Allen Blash Foundation (ABF I understand that this program is designed to provide athletic development, mentorship, and life skills through basketball and related activities. I hereby consent to the following terms:
I grant permission for myself/my child to participate in all ABF Hoops Initiative activities, including but not limited to basketball practices, games, training sessions, workshops, and otl recreational activities organized by ABF.
In the event of an emergency, I authorize the ABF staff or volunteers to obtain medical treatment for myself/my child if I am unable to be reached. I understand that I will be responsible for any medical expenses incurred.
I consent to the use of photographs, video recordings, and audio recordings of myself/my child taken during program activities for promotional, educational, and fundraising purposes by ABF. I understand that these materials may be used in social media, newsletters, websites, and oth promotional materials.
I release The Allen Blash Foundation, its staff, volunteers, coaches, and affiliates from any liability for injuries, accidents, or any other incidents that may occur during participation in the ABF Hoops Initiative. I understand that all reasonable efforts will be made to ensure a safe ar positive environment for all participants.
I understand that ABF may collect personal data, such as contact information and participation records, for program administration and reporting purposes. I consent to the collection and us of this information with the understanding that it will be kept confidential and not shared with third parties without my permission.
I acknowledge that myself/my child is expected to follow the rules and guidelines set forth bythe ABF Hoops Initiative to ensure a safe, respectful, and productive environment for all participants. Failure to adhere to these expectations may result in dismissal from the program
