I, the undersigned participant, request voluntary participation in Mindfold Experiences events.
I acknowledge that Mindfold Experiences events may include:
- Blindfolded interaction with surrounding.
- Loud gong meditation
- Breathing exercises
- Spiritual experiences that may alter consciousness
- Activities with potential for physical injury
- Gatherings in private residences
I fully understand that my participation in these activities involves risks of serious bodily injury, including permanent disability, paralysis, and death. These risks may be caused by my own actions or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "Releasees" named below.
I hereby release, discharge, and covenant not to sue Mindfold Experiences, its organizers, volunteers, and agents (each considered "Releasees" herein) from all liability, claims, demands, losses, or damages caused or alleged to be caused in whole or in part by the negligence of the Releasees.
I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or cost which may incur as a result of my participation in Mindfold Experiences events.
I assume full responsibility for any risk of bodily injury, death, or property damage arising out of or related to the event, whether caused by the negligence of Releasees or otherwise.
I grant Mindfold Experiences the right to use my likeness, voice, and image in any photographs, videos, or recordings made during events for promotional purposes.
I acknowledge that Mindfold Experiences does not carry participant insurance and that I am responsible for my own medical expenses.
I agree that this Release applies to all future Mindfold Experiences events I attend until revoked by me in writing.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
