Please follow this link below to also complete the pet travel form at least 2 days prior to your appointment. The information you provide will allow us to start the process of a Certified Veterinary Inspection (CVI) for traveling with your pet. Please note that most Health Certificates are valid for 10-30 days, depending on how and where you are traveling.
