DISCLOSURE FOR CONSUMER REPORTS

 

In connection with your employment or independent contractor application, GNS TRUCKING INC. may order one or moreconsumer report(s) (commonly known as "background reports" or "background checks") about you from one or moreconsumer reporting agencies. If you are hired or engaged as an independent contractor, additional consumer reports may be obtained in connection with and throughout your employment for employment purposes or for the legitimate business purpose of evaluating you as an owner-operator. To the extent allowed by law, the consumer reports may include information concerning your character, general reputation, personal characteristics, mode of living, drug and alcohol test results, motor vehicle records, driving records, criminal history, public court records, employment history (including names and dates of previous employers, reason for termination of employment, work experience, and accidents), social security number validation, education, licensure, or verification of other information supplied by you. Such reports may be obtained from private and public record sources, including sanctions databases, CDLIS (including but not limited to CDLIS Central Site, CDLIS Master Pointer Record data and your driver record from the jurisdiction identified in the CDLIS data, in accordance with applicable state law and the Driver Privacy Protection Act), former employers, public court records, and federal, state, and other government agencies that maintain such records. I AUTHORIZE GNS TRUCKING INC (1140 E KIMBERLY RD STE 230 DAVENPORT, IA 52807 ) WITHOUT RESERVATION, TO RELEASE THEABOVE INFORMATION.I have the right to make a request to the CRA, upon proper identification, to request the nature and substance of all information in its files on me at the time of my request, including the sources of information: and the recipients of any reports on me preceding my request. I hereby consent to your obtaining the above information from the CRA, and I agree that such information which the CRA has or obtains, and my employment history with you if I am hired, will be supplied by the CRA to other companies which subscribe to the CRA's services. In accordance with the provisions of Section 604(b)(2)(A) of the Fair Credit Reporting Act. Public Law 91-508 as amendedby the Consumer Credit Reporting Act of 1996 (Title II. Subtitle D. Chapter I. of Public Law 104-208). you are beinginformed that reports verifying your driving record may obtained on you for employment purposes. These reports are required by Sections 382.413.391.23. and 391.25 of the Federal Motor Carrier Safety Regulations. I authorize GNS TRUCKING INC to obtain one or more consumer report(s) or investigative consumer report(s) about me. If hired or engaged as an owner-operator (independent contractor), I understand this authorization shall remain on file and shall serve as ongoing authorization for additional consumer reports or investigative consumer reports to be obtained from any consumer reporting agency at any time during my employment or contract period without asking me for authorization again.

MOTOR VEHICLE RECORD RELEASE & AUTHORIZATION

 

 

I, ______________________________________ do hereby authorize and allow GNS TRUCKING INC to run my Motor Vehicle Record. I understand the company will use these records to evaluate my suitability to fulfill driving duties that may be related to the position for which I am applying. I also consent to the review, evaluation, and other use of any Motor Vehicle Record I may have provided to the company.
This authorization is good until revoked by me in writing.
The authorization is given in satisfaction of Public Law 18 USC 2721 et. Seq Federal Drivers Privacy Protection Act”, and is intended to constitute written consent as required by this act.

 

By signing below, I agree that have read and understand the foregoing Disclosure, and authorize the Company to obtain my Motor Vehicle Record for employment purposes and, if I am hired or I have already been hired, from time to
time while I am employed by Company without further authorization from me. I further authorize the Company to share the information with any person involved in the employment decision about me.

DRUG AND ALCOHOL TESTING CONSENT AND AUTHORIZATION

I hereby consent to submit to a Drug or Alcohol Test and to furnish a sample of my urine, breath, and/or blood for analysis, as shall be determined by (Company Name) GNS TRUCKING INC. in order to meet with policy regarding the selectionof applicants for employment.

I further authorize and give full permission to have the Company and/or its authorized agents and physicians to send the specimen or specimens so collected to a laboratory for a screening test for the presence of any prohibited substances under the policy, and for the laboratory or other testing facility to release any and all documentation relating to such test to the Company.

I further agree to and hereby authorize the release of the results of said tests to the Company.
I understand that it is the current use of illegal drugs that would prohibit me from being employed at this Company.
I further agree that a reproduced copy of this Pre-Employment Consent and Release Form shall have the same force and effect as the original.
I have carefully read the foregoing and fully understand its contents. I acknowledge that signing this Consent and Release Form is a voluntary act of my part and I have not been coerced into signing this document by anyone.
