2025 Individual Reconditioning Order Form

THANK YOU FOR CHOSING MERCURY SPORTS TO RECONDITION YOUR PERSONAL FOOTBALL HELMET.

Please completely fill out this form. Be accurate and specific. If you have questions please reach out to us prior to submitting form.
Helmets that are dated 2015 or older are automatically rejected so do not send in for reconditioning.
Pricing on this form are base prices. Any parts that are missing or damaged will be replaced and that cost will be added to the base price.
When your helmet is complete, you will receive an invoice by email and will include the payment options. Payment must be received before your helmet will ship back to you.
We DO NOT recondition VICIS or LIGHT helmet
