    All Volunteers must fill out this membership application to be apart of FAU's Global Medical Brigade Chapter. 

  • Application Instructions:

    1. Review trip details and the individual donation goal.

    2. Fill out the personal information section.

    3. Read the Brigade agreement form and sign your name.

    4. Once you have submitted this application you will be emailed a website link which you must register yourself on the FAU's brigade website. 

  • Trip Details:

    Dates: August 3rd- August 9th 2025 to Guatemala 

    Flight Info: MIA-> GUA (La Aurora, Guatemala City) 03-Aug 2025 8:35AM-9:20PM

    GUA-> MIA 09-Aug 2025 2:35PM-7:25PM

     

    Donation Goal PER Volunteer: $2,150.00

    (NON REFUNDABLE) 

    *Price is subject to change based on flight price*

    First payment ($250.00) due: March, 24th 2025

    Second payment ($1,250.00) due: May, 21st 2025

    Final payment due: June, 20th 2025  

    Donation Goal Includes:

    • Airfare
    • Ground Transportation
    • Meals 
    • Lodging
    • Emergency Insurance 
    • Medications
    • Doctors
    • Translators
    • One carry-on bag

    Donation Goal DOES NOT Include:

    • Baggage Fees (1 bag $35, 2nd bag $45)

  • Please read all of the following rules and expectations.

    In order to be elligable to attend this Brigade, you MUST agree to the following statements and sign below stating that you will abide by these regulations.

    If you are unwilling to abide by these rules, you will not be allowed to go on this trip.

    1. I will abide to all the health travel regulation requirements to enter Guatemala (up to date vaccinations).

    2. I agree that the total donation goal is $2,150 and the first payment of $250 is due March, 24 2025 and is NON REFUNDABLE 

    3. I agree that if I do not meet my donation goal through fundraising events I will pay the amount out of pocket.

    4. I agree that I will have a valid passport prior to departure date.

    5. I agree to pay the $30.00 membership dues.
