Nomination Guidelines for Youth Alive Clubs and Prayer Tables

This form is to nominate students who have started a Youth Alive Club or Prayer Table at their school between Momentum 2023 and Momentum 2024.

Please note:
- This nomination is for the student who initiated the club or table, not for someone who simply attends.
- Students who have already been honored at the Honors Banquet for starting a Youth Alive Club or Prayer Table are not eligible for this nomination. However, if the student has attended the banquet for a Speed the Light giving award and has now started a Youth Alive Club or Prayer Table, they can be nominated.
The Honors Banquet will be held at Momentum 2024, on Friday evening at 5:15 pm. Further details will be provided soon.
