Field Surrender Form

I hereby certify that I am the rightful owner/caretaker/custodian/guardian of the cat(s) who is/are the subject of this Cat Surrender Form, hereinafter referred to as “the cat”. I hereby surrender any and all rights to the cat. I certify that no other person has a right of property to the cat; if the cat is a stray, I have reported the cat as found to local lost/found pet resources. I agree that all future decisions regarding adoption, veterinary care and general care are the sole responsibility and at the sole discretion of The Cat House. I authorize transfer of any microchip registration to the cat’s next owner or The Cat House. I authorize release of any veterinary records to The Cat House. Should the situation arise that I would like to reclaim the cat you surrender, I will be subject to the standard adoption application process of the Cat House, including a screening interview and payment of an adoption fee. I also understand that, while The Cat House is a no-kill shelter, there are circumstances that necessitate the use of humane euthanasia. While not an all-inclusive list, euthanasia would be considered in cases of terminal illnesses that result in a substantial loss of quality of life or animals that are overly aggressive or otherwise pose a substantial risk to other animals or the public. The Cat House reserves the right to make all decisions related to the surrendered cat(s) and euthanasia. The Cat House is primarily funded by donations. A donation of $50 per cat surrendered is suggested to help offset costs of food, general care, and veterinary care. Larger donations are much appreciated.
