2025-2026 Competition Student Admission & Waiver Policy
ADMISSION POLICY
To attend any Hospitality Institute of Ohio events, each student must complete and return this form acknowledging that they understand the admission policy. This completed form must be submitted prior to the event.
Hospitality Institute of Ohio seeks to prepare students for professional careers where certain standards of dress are required for employment.
· Refer to event-specific rules for uniform guidelines.
· Students must be currently participating in the 2025-2026 Ohio ProStart program or Ohio Hospitality CTSO program to compete or to be part of an observation team.
· A ratio of 1 chaperone/instructor per 5 students is required.
· Inappropriate behavior will not be permitted at the event venue at any time. Students are expected to behave in a professional manner and represent themselves and their school appropriately.
· No lounging, laying or sleeping on venue floors or furniture.
Individuals found violating any of these rules may be asked to leave the event along with the chaperone with whom they arrived. No refunds will be given, and the team may be disqualified from the competition. The final decision will be made by the Hospitality Institute of Ohio Executive Director.
PARTICIPANT WAIVER & RELEASE FORM
Hospitality Institute of Ohio and its affiliated organizations recognize that the safety of all participants and guests at Ohio ProStart and Ohio Hospitality CTSO events is a top priority. Hospitality Institute of Ohio is committed to following the U.S. Centers for Disease Control and Prevention (“CDC”), local municipalities and event venue safety directives and recommendations.
The undersigned willingly and knowingly assumes all responsibility for all risk of bodily injury (including illness, physical injury, and death), property loss, or damage that you may sustain at any event-related activity. You understand that Hospitality Institute of Ohio assumes no responsibility or obligation to provide financial assistance or other assistance, including but not limited to, medical, health, or disability insurance, in the event of an accident, injury, illness, death or property damage or loss.
I/we hold Hospitality Institute of Ohio, its affiliates, successors, assigns, directors, employees and agents harmless from any and all liability or claims, which may arise out of or in connection with participation of my child in this event. I/we fully understand that participants are to abide by all rules and regulations governing conduct during the event. Any violation of these rules and regulations may result in the participant being sent home at the expense of the participant and/or his/her parent/guardian.
Misconduct, which includes, but is not limited to, any drug or alcohol use, at the event, during the event or in activities or locations related to or surrounding the event, may disqualify a participant or the participant’s team from further participation or awards. Misconduct will be investigated as Hospitality Institute of Ohio deems appropriate. Decisions and sanctions concerning misconduct are at the sole discretion of Hospitality Institute of Ohio and are final. By entering the event, the participant and his/her parent/guardian and the team accept these terms and conditions.
I/we hereby consent that Hospitality Institute of Ohio, its affiliates, successors, assigns, directors, employees and agents shall have the right to publish or use any photographs, movie films, video tapes, digital images and/or sound recordings (including submitted text or any part thereof) they have taken or made of me or in which I/we have been included, for publicity, advertising, Internet usage or any other lawful purpose in conjunction with my own or a fictitious name, or in reproductions thereof. I/we hereby release, discharge and agree to save harmless Hospitality Institute of Ohio, its affiliates, successors, assigns, directors, employees and agents from all liability as a result of any blurring, distortion, alteration, or optical illusion, whether intentional or otherwise, that may occur, including, but not limited to, any claims for libel or invasion of privacy. I/we hereby waive all claims for any compensation for use or for damages. I/we hereby waive any right that I/we may have to inspect and/or approve the finished product or the use to which it may be applied.
I/we hereby warrant that I/we have every right to contract in my/our own name. I/we have read the above authorization, waiver, release, and agreement, prior to its execution, and I/we are fully familiar with the contents. I/we hereby further warrant that this document is binding upon my/our heirs, administrators, personal representatives and assigns.
This form must be completed and signed by both the participant and the participant’s parent or guardian before the participant is eligible to participate in the event. Failure to timely complete and submit this form will prevent the participant from participating in the event. I/we have read, understand and agree to comply with the terms and conditions set forth above.
