    City of Forest Day (CoFD) is possible because of event hosts like you. Please help us improve CoFD for future partners and participants by completing this short survey (about 5 mins).

  • If you organized more than one event, please fill out this evaluation form separately for each event.

    Information provided in this survey will be used by the CoFD Steering Committee for internal planning purposes and as a baseline for future events. Anecdotes may be used for public facing reports and in promotional materials for future CoFD events.
