I PARENT/LEGAL GUARDIAN* , the parent/legal guardian of ATHLETE*
I. TRAVEL PERMISSION
I, the undersigned parent/legal guardian of the above-named child, hereby grant permission for my child to travel under the supervision of DC United Sports Club Inc. and its appointed staff, coaches, or volunteers to any team-related events, games, practices, or other club-approved activities that may require transportation.
I understand that all appointed coaches and staff will take every reasonable precaution to ensure my child's safety during transportation and related activities. I acknowledge and accept the risks involved in such travel and agree not to hold DC United Sports Club Inc., its officers, employees, agents, or volunteers liable for any injuries, losses, or damages that may occur during or as a result of travel.
II. PHOTO/VIDEO RELEASE
I grant permission to DC United Sports Club Inc. to take photographs, video, or audio recordings of my child during team events, practices, games, or related activities. I agree that these materials may be used for promotional, media, or marketing purposes, including but not limited to use on the Club's website, social media, newsletters, and printed materials.
I understand that no personal information (e.g., full name, address) will be disclosed without additional consent.
III. RELEASE OF LIABILITY
In consideration of my child’s participation, I hereby waive, release, and discharge DC United Sports Club Inc, Urban Bridge, Any Flag Football Leagues Affiliated with DC United Sports Club Inc and its representatives from any and all claims or liabilities arising from travel, participation, or media use, including but not limited to personal injury, accidents, or loss of property.
