I, {name}, hereby apply for membership in Grace Baptist Church of Richmond Hill in accordance with the following:
- I have placed my faith in Jesus Christ alone as my Lord and Saviour, believing that his death, burial and resurrection made full atonement for my sins.
- I have publicly confessed my faith in Jesus Christ through baptism in water by immersion.
- Having received and reviewed the General Operating By-Law No. 5 of Grace Baptist Church, along with any policy statements adopted by the church, I make the following commitments:
  - I am in agreement with the Statement of Faith (Article 1.03), and should I come to question any of those beliefs in the future, I will inform a member of the Board of Elders or the Pastor of my question(s).
  - I commit myself to live in obedience to the Scripture; to be subject to the authority of the Church as expressed in the above-mentioned By-law and any policy statements adopted by the church from time-to-time, and to refrain from supporting any lifestyle or activity which the church deems morally inappropriate.
  - Should the Board of Elders or Pastor recommend it, I agree to complete a preparatory course before proceeding further with this applicaiton for Church membership.
- I have provided a written testimony of how I came to trust in Jesus Christ as Lord and Saviour. This testimony includes the details of my baptism (e.g. date, place and person performing the baptism).
