How do you prefer to receive your order? Selection required:*

Delivery - Standard ground delivery to one address outside of RI. For multiple addresses you must order separately on new form.Pick-up in Warren or delivery within RI (free) - Apply code "BITEP" at checkout below to waive shipping fees for all pick-up purchases.
IMPORTANT: All items have a preset minimum quantity of "1" input. However, an item is not added to your order until the checkbox in the top left corner of the product is selected. Once you select a product, the item quantity will be attributed to your order and the subtotal will reflect the price of selected products.

Multiple Boxes: Only one delivery method or shipping address can be selected per order. If you have multiple orders or shipping requirements, you must complete this form for each unique request.
