Please read the following statement.

I hereby grant permission to Connections its affiliated schools, programs, successors, assigns, licensees, contractors and those acting with/on behalf of Connections to interview, identify, photograph, and/or film me, if I am a student over the age of eighteen (18) or an emancipated minor, or my student and use Connections Media Property (as defined above), in all forms for inclusion in Connections’ internal and external publications and promotional materials, including, but not limited to, corporate, non-corporate and internet communications, such as social media, and video and print media , for display in its offices, on its Website and in its advertising, promotion or any other related purpose whatsoever and in any format, including advertising, trade or commercial purposes throughout the world and in perpetuity.

I understand that Connections may edit and modify Connections Media Property, including incorporating Connections Media Property (in whole or in part) into other works, which may or may not include other students’ Connections Media Property (in whole or in part). I waive the right to inspect or approve edited versions of the Connections Media Property or other works in which the Connections Media Property is incorporated (in whole or in part).

I agree to waive the right to receive any compensation or royalties as a result of the use of any Connections Media Property by Connections. Connections shall have complete ownership of all Connections Media Property covered by this release (including video footage, audio, moving film footage and photographs) and to the extent it is ever determined by a competent authority that Connections does not enjoy such ownership rights, I hereby grant Connections an irrevocable, perpetual, royalty free, transferable, worldwide license to make use of the Connections Media Property in any manner, including but not limited to, the rights of performance, display, reproduction, licensing to third parties, and distribution of all Connections Media Property, and the rights to create, perform, display and distribute derivative works thereof.

By granting permission, I release Connections from any claims that may arise regarding the use of Connections Media Property, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, or copyright. Connections is permitted, although not obligated, to include my name or my student’s name as a credit in connection with the image. Connections is not obligated to utilize any of the rights granted in this release.

I understand that if I withhold my consent, Connections shall not use any of my or my student’s Connections Media Property in any form, for any reason unless and until it receives my consent to do so.

I understand that if in the future I change my Connections Media Property release election to permit the use of Connections Media Property, such permission will be retroactive to the date of my or my student’s enrollment.

I understand I may revoke my consent at anytime. However, my decision to revoke will only be applicable to Connections Media Property created after the date of revocation. My consent to use Connections Media Property given prior to the date of revocation is not revocable.
