If the Letter of Intent is accepted, interested parties are asked to submit a full application of no more than eight (8) single-spaced pages. An electronic (PDF) copy of the application in one document is required and should include the following:
Colleges/Universities
- Full description of the plan to develop an APRT program at the master’s degree or higher.
- Needs assessment (RTs, physicians, employers)
- Description of the curriculum proposal development process
- Description of the curriculum proposal approval process
- Demonstrated effort of collaboration with state affiliate
- Letter of support from College/University Leadership (e.g., Associate Dean, Dean, Associate Provost, Provost)
- Letters of support from potential employers (preferred but not required)
- Physician offices (physician groups)
- Academic departments
- Medical directors (clinical or academic)
- Clinical departments in non-academic facilities
- Detailed proposed budget and justification
- Timeline for curriculum proposal development
State Affiliates
- Full description of plan to develop advocacy efforts for legislative or regulatory initiatives for APRT licensure including any current efforts underway.
- Minutes from state affiliate documenting the Board of Directors approval for the grant application
- Patient needs assessment (e.g., projected need for pulmonary physicians, NPs, PAs)
- Draft of proposed legislation or regulatory language, or the need for a proposed working group to establish language (preferred but not required)
- Detailed proposed budget and justification
- Timeline for advocacy proposal development
- Additional supporting documentation (preferred but not required):
- Letter of support from Legislative Sponsor(s)
- Letter of support from licensing agency/board
- Letter of support from state society medical advisor
Applications of any amount up to $30,000 will be accepted based on available funding. Please note that the amount of the grant will cover direct expenses only and will not cover indirect overhead costs associated with administrative operations.
