I, the above-named participant, am 18 years of age or older and have voluntarily applied to participate in the above described Activity or Trip and for Hope International Missions (HIM) to furnish any transportation, food, and lodging for me. This includes consent to be driven in private vehicles by HIM representatives and the use of public or private transportation provided or organized by HIM representatives in conveyances such as taxis, trains, buses, airplanes, subways, etc.

I GIVE PERMISSION to be treated in case of medical emergency. In the event of a medical emergency and my emergency contact person cannot be reached or there is no time or opportunity to reach that person given the circumstances (as determined in the sole discretion of HIM trip leadership), I authorize HIM’s staff or volunteers, and/or emergency and medical personnel to make emergency medical decisions for me. I ACKNOWLEDGE that HIM does not provide any health insurance covering me during the Activity or Trip, and I further acknowledge that it is my responsibility to obtain health insurance covering me. I agree to accept the sole responsibility for the cost of my medical care arising out of or related to said Activity or Trip.

I ALSO GRANT PERMISSION to HIM and its representatives, contractors, employees and volunteers acting on behalf of HIM, to take and/or use, copyright, publish, edit, crop or treat images or likenesses of me, including photographs, videos or otherwise, for any lawful use on HIM’s website, social media pages, blogs, or in other official HIM printed or electronic publications without further consideration. I understand that this consent and release will operate in full force and effect indefinitely. I understand that should photographs or videos of me be used on HIM-owned or operated websites or webpages, they may be available for download.

I UNDERSTAND AND AGREE that should it be necessary for me to return home due to medical reasons, disciplinary action or otherwise, I hereby consent to pay or reimburse HIM for all related transportation costs.

I ALSO AGREE to reimburse HIM for any trip-related expenses paid in good faith by HIM on my behalf, provided HIM has had my written or verbal approval prior to making the purchase or expenditure, should I not participate in the funded event (airline tickets, seat of event, travel expenses, or whatever HIM has funded). 

I ALSO AGREE TO HOLD HARMLESS AND TO INDEMNIFY HIM and its agents for any liability sustained by HIM or its agents as a result of my negligent or intentional act or failure to act.    

I UNDERSTAND that I have no expectation of privacy in any item that is HIM-owned, including but not limited to any electronic devices or online services, and agree that HIM authorities have the right to conduct random inspections of any HIM-owned items to protect Activity or Trip participants’ health, safety, and welfare.

I UNDERSTAND that in the event of a kidnapping and/or hostage situation, HIM will take all reasonable steps to secure my release and/or the release of any trip member held hostage and/or detained. I ACKNOWLEDGE AND ACCEPT that HIM will not pay ransom or participate in third-party payments of ransom.

I UNDERSTAND AND AGREE TO ASSUME ALL THE RISKS which may be encountered during the Activity or Trip sponsored by HIM that I will be attending pursuant to this form, including transportation to and from said events. RISKS THAT MAY BE ENCOUNTERED on this trip may include but are not limited to loss of, stolen, or damage to personal property; injury or death resulting from vehicle, plane, mechanical, pedestrian, or any other accident; medical illnesses or injuries; infections, including bacterial, fungal, parasitic, viral, HIV, etc.; post-traumatic stress, depression, or mental anguish; uprisings within an area in which I will be traveling; imprisonment; difficulty entering or leaving an area; being a victim of deceit or fraud; and permanent or temporary disability or death from natural or unnatural causes. I understand that any of the forementioned situations could arise even if HIM and I comply with all laws, regulations, recommendations of HIM, any other sponsoring organizations, and the areas in which I will be traveling, and even if HIM and I provide adequate and reasonable personal protection against natural elements/conditions or diseases.

In consideration of being permitted to participate in this trip and other valuable consideration, the receipt of which is acknowledged, I AGREE TO RELEASE, DEFEND, INDEMNIFY, AND HOLD HARMLESS HIM and its agents from any and all past, present, and future, known and unknown liabilities, actions, causes of action, claims, expenses, personal injuries, and damages, INCLUDING THOSE CAUSED BY THE NEGLIGENCE OR FAULT OF HIM AND ITS AGENTS, and including without limitation, interest, penalties, court costs, attorney's fees and expenses resulting from or on account of injury to myself or my property in connection with any event anticipated by this form.

I FURTHER RELEASE any and all claims brought by or through me, including claims for loss of consortium and all similar claims based on relationships with other people. I EXPRESSLY AGREE that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted in the State of Florida and that if any portion hereof is held invalid, it is agreed that the remainder shall, notwithstanding, continue in full legal force and effect. I ALSO AGREE that any controversy or claim, by or through the party signing this release, arising out of or relating to the activities anticipated by this form shall be settled by binding Christian arbitration conducted by the National Center for Life and Liberty or another Christian arbitrator, and judgment on the award may be entered in any court having jurisdiction thereof. This release contains the entire agreement between the parties.

The theological statement to which FEA subscribes and to which it expects all missionaries to affirm their allegiance, is that of the conservative Wesleyan-Arminian position and embraces the following: 

1. The Bible, both Old and New Testament, is the inspired Word of God and is infallible and inerrant in the original autographs. 
2. There is one eternal God existing in three persons: God the Father, God the Son, and God the Holy Spirit. 
3. Jesus Christ is the eternal Son of God. He was born of the Virgin Mary, died for the sins of mankind on the cross, was buried, and on the third day rose again. He is now seated at the right hand of the throne of God in heaven from whence He is coming again to receive the Church as His bride. 
4. The Holy Spirit is the Third Person of the Holy Trinity, and is the operating agent in the redemption of mankind, the superintendent of the Church, and the abiding Comforter of the children of God. 
5. Man was created by the direct act of God and was made in the image and likeness of God, but through transgression fell and became depraved in nature and sinful in conduct. 
6. Salvation is by faith in the atoning sacrifice of Jesus Christ. 
7. Regeneration is the act of the Holy Spirit whereby the penitent, trusting sinner is renewed in nature, changed in conduct, and brought into the family of God. 
8. Entire sanctification is a second work of divine grace, subsequent to regeneration, wrought in the heart of the fully yielded, trusting child of God; whereby the heart is purified by faith and filled with the Holy Spirit. 
9. There will be a resurrection of the dead, both of the saved and unsaved; the saved to eternal bliss in the presence of God, and the unsaved to everlasting damnation. 

Hope International Missions desires to reflect a consistent, conservative lifestyle as pleasing to God. To ensure consistency around the world, we request our teams be in accordance with the following policies.  

Dress Code

• No immodest dress such as see-through, low-cut shirts or blouses, or tight-fitting clothing may be worn.
• Clothing should not have any type of offensive slogans or pictures.
• Shirt sleeves are to be of modest length and fit.
• All women should wear dresses or skirts that extend below the bottom of the kneecap when sitting or standing. Any splits must be below the knee.
• Shorts, pants/jeans may not be worn by women.
• Shorts may not be worn by men.
• No make-up or jewelry may be worn. This includes wedding bands.
• Men’s hair must be neatly trimmed.
• Women must have their hair fixed neatly.

Miscellaneous Guidelines

• Smoking, drinking, non-prescription drugs, profanity, suggestive language, or gambling are not permitted.
• No disruptive conduct of any kind is permitted.
• Nothing considered incompatible with the purpose of the mission trip may be taken.
