1.) CANCELLATIONS
(a) The personal trainer reserves the right to charge you for a training session if you do not show up for that session. This session will be classified as a "NO SHOW SESSION" and will be deducted from your session balance.
If you become aware that you cannot make a scheduled appointment, contact your trainer.
(b) This policy allows a member to reschedule unused sessions, provided they give at least 24 hours' notice. The rescheduling of these unused sessions is limited to a maximum of one week from the expiry of your plan, and the number of rescheduled sessions cannot exceed the weekly session limit set by the member's plan.
For example:
If a member has a Platinum plan with 5 sessions per week, they can only move a maximum of 5 unused sessions. They cannot move more sessions than their weekly plan allows.
(c) In the event that a member cancels their personal training plan, any remaining unused sessions cannot be converted or transferred for use as general gym membership or access to other services. All unused sessions will be forfeited upon cancellation, and no refunds will be provided for these sessions.
(d) In the event that the assigned personal trainer is unavailable, another trainer will be assigned to conduct the session. If no other trainer is available at the scheduled time, the session will be rescheduled to a mutually agreed-upon date and time.
2.) TRANSFER
The member is not allowed to transfer any pending sessions to another individual or another service provided by the gym.
3.) MEMBERSHIP FREEZE
The member acknowledge and agree that this membership is non-freezable for any reason. This includes, but is not limited to, personal circumstances, travel, medical conditions, or other unforeseen events. The membership term must continue uninterrupted from the start date until the end date as specified. No requests for freezing the membership will be accepted under any circumstances.
4.) SCHEDULE
The member must at all times adhere to the schedule that was mutually agreed to by both the trainer and the member.
5.) LATE ARRIVALS
Each session shall be 45 minutes -1 hour in duration as initially agreed upon by the client and trainer. Sessions will not be extended due to the lateness of the client or due to interruptions caused by the client.
6.) LIABILITY
In recognition of the possible dangers connected with any physical activity, the client hereby knowingly and voluntarily waives any right of cause of action of any kind whatosever arising from any liability which may or could accrue to the gym and its trainers.
Failure to use all personal training sessions or follow program will not relieve client payment. If original trainer is unable to complete session, Evolve Fitness has the right to assign another trainer. No refunds will be given.
6.) ALL THE INFORMATION I HAVE GIVEN IS CORRECT
All the information on this form is correct and to the best of my knowledge. I have sought and followed any necessary medical advice. I understand that all the information given will be kept confidential.
