CONFIDENTIALITY AGREEMENT
Purpose of Confidentiality:
The purpose of maintaining confidentiality within the group is to create a safe and trusting environment for open discussion and sharing.
Scope of Confidentiality:
All information shared within the group, including personal stories, experiences, and discussions, is considered confidential.
Exceptions to Confidentiality:
Confidentiality may be breached if there is a risk of harm to oneself or others, or if required by law.
Participant Agreement:
I, acknowledge and agree to the following:
I will respect the confidentiality of all information shared within the group.
I understand the exceptions to confidentiality as outlined above.
I will not disclose any information shared by fellow group members outside of the group setting.
Facilitator Responsibilities
Facilitators will:
Ensure a safe and confidential environment for group discussions.
Discuss and reinforce the importance of confidentiality during group sessions.
Take appropriate action if confidentiality is breached.
Duration of Agreement:
This confidentiality agreement is effective for the duration of my participation in the group.
