SignatureSubscription Agreement and Terms of Service This Subscription Agreement ("Agreement") is entered into by and between [The Milestone Market] ("Company") and the undersigned subscriber ("Subscriber"). 1. Subscription Services The Company agrees to provide the Subscriber with a monthly delivery and installation of balloon decorations or a DIY balloon kit ("Services") as per the selected subscription plan. The Services will include monthly themed balloons, installation services, DIY kits with instructions and access to member portal. 2. Subscription Plans Subscribers can choose from the following plans: Monthly Delivery & Installation: $197/month DIY Kit Subscription: $47/month 3. Term and Termination This Agreement shall commence on the date of acceptance by the Subscriber and shall continue on a monthly basis until terminated by either party. Either party may terminate this Agreement with [30] days written notice to the other party. In the event of termination, the Subscriber will receive the last month's Services, and no further charges will be incurred. 4. Payment Terms The Subscriber agrees to pay the monthly subscription fee as specified at the time of signup. Payments will be automatically charged to the payment method provided by the Subscriber on the [day of the month] of each month. Any late payments may incur a 15% late fee after 15 days past due. 5. Delivery and Installation The Company will deliver balloons to the address provided by the Subscriber on the agreed date each month. Installation services will be conducted by trained professionals of the Company unless the DIY kit option is selected. 6. Subscriber Obligations The Subscriber agrees to provide accurate and complete information for delivery and installation. The Subscriber is responsible for ensuring that the installation location is accessible and safe for the Company’s personnel. 7. Liability and Indemnification The Company shall not be liable for any indirect, incidental, or consequential damages arising from the use of Services. The Subscriber agrees to indemnify and hold harmless the Company from any claims or damages arising from the Subscriber's use of the Services. 8. Governing Law This Agreement shall be governed by and construed in accordance with the laws of Kansas. 9. Modifications The Company reserves the right to modify the terms of this Agreement at any time. Subscribers will be notified of any changes and will have the option to accept the revised terms. 10. Acceptance By signing below, the Subscriber acknowledges that they have read and agree to the terms and conditions of this Agreement.
