Community Dreams

Originally established as a private health conversion foundation in 2003, Pottstown Area Health & Wellness Foundation transitioned to a public community foundation on July 1, 2023. We are excited to serve the community in a new way. In addition to continuing with our core grantmaking, we are partnering with individuals, businesses and organizations to assist them in accomplishing their charitable goals. Oftentimes, donors will ask us what are some projects and programs that require financial support in a community to address the pressing issues they care most deeply about. This is where we need your help!

We invite you to complete this form to help us align the dreams of our donors with the dreams of the nonprofit community. These could be short term projects that are in need of immediate financial support, to long term dream projects that could be accomplished if funding was not an obstacle. These can be shovel ready projects, projects in the planning stages, or preliminary concepts. Either way, our goal is help you carry out your mission and vision for the Pottstown area. You will have the option to list up to three programs or projects for your organization that we could share with our donors to be supported through endowment funds, capital gifts, or programming dollar contributions.

Although we cannot guarantee that your program or project will be supported, we believe that this will help inspire existing and potential donors. When we find that a donor is interested in your project, we will reach out to you if more information is needed. We look forward to continuing our partnership in a new way!
