Important Registration & Program Information
- Cost is $40 per family. Please make your check payable to OSLC with the word KidZone in the memo line. You can send the payment with your child on the first day or pay it when you pick your child up. The fee covers supplies and bus transportation provided by Prairie Hills Transit. Limited scholarships may be available. Please contact Beth at oslcyouth@rushmore.com if this is a need.
- YOU MUST COMPLETE A "PRAIRIE HILLS TRANSPORTATION" FORM if your student is going to ride the bus AT ALL, including going to the bowling alley in May. The form is at the end of this form or you can email me at oslcyouth@rushmore.com and I can email the form to you.
- Registration is limited to 30 students and will close when that number has been reached.
- Pick Up Time: All Students must be signed out and picked up by a responsible adult. Please do your very best to pick up your child by 5:00pm. THANK YOU IN ADVANCE!
- SNACKS & Allergies: We serve snacks a minimum of two times during KidZone. We have one larger snack of a protein, a fruit and a carb. The second snack is usually a granola bar or rice krispie treat.
- We do have limited dairy free/gluten free snacks available. If your student requires special dietary items, please send a generous snack for them.

Follow this link to the Prairie Hills Transportation Form. The form is at the very bottom of the page, marked "Kidzone Trasportation Form". You will need to complete a form for each child that will be riding the bus.

You can download the form, fill it out and return it to PHT. You can take a picture of the completed form and email it.
