My student is interested in participating in the following sport. I understand that the following will be required:
- Students will be expected to abide by the LBA Student Handbook and Statement of Cooperation.
- An athletic participation fee of $60 to cover team expenses and equipment. This will be billed to your account after the first week of practice.
- An annual sports physical must be on completed and filed in the academy office to ensure your students health and safety on the field. (Students who played soccer or volleyball will already have those on file and will not need to renew for the basketball season.)
