KADIMA is seeking participants for a clinical study of the
investigational drug psilocybin for Major Depressive Disorder (MDD) .

The principal investigator at our site is DAVID FEIFEL, MD, Ph.D.

The following survey will assess your current eligibility to participate in this trial. This survey should take approximately 5 minutes to complete.
Please answer as honestly and as completely as possible. Responses will be verified at the next stage through your medical records and/or in-person assessments.
