  • Evaluation Key

    5- Exceeds Expectations- Consistently performs at a high level above expectations of the role with little or no supervision

    4- Above Expectations- Performs all tasks based on expectations of the role or above expectation with little supervision.

    3- Meets Expectations- Performs all tasks required at the expectation level of the role with appropriate level of supervision.

    2- Below Expectations- Performs tasks required of the role below expectations of the roles, requires more supervision. May require additional training

    1 Does Not Meet Expectations- Does not perform required tasks of the role at the expectation of role, requires performance plan.
