Please use this form to add your (sponsor) name to the waiting list.  We are currently limited to 880 crosses/flags.  Names will be placed on the waiting list in the order they are received.  Once your name is at the top of the waiting list, we will contact you and offer you a spot to honor your Hero.  You won't need to pay until you are contacted.  Currently, the cost is $50 for the first two years.
