1. Vaccination Requirements
   - Current vaccinations required (e.g., rabies, distemper, Bordetella).
   - You can send/text me pictures of records
2. Health Requirements
   - Dogs must be free from fleas, ticks, and contagious illnesses.
   - Any existing medical conditions must be disclosed.
3. Pet Identification
   - All pets should wear a collar with ID tags.
   - Microchipping is encouraged.
4. Check-In/Check-Out Procedures
   - You can come in anytime to pick up and drop off your dogs/cats
   - Late check-out policy (if applicable).
5. Neutered/spayed
   - We do not accept non-neutered dogs or cats
   - however, we are welcoming non-spayed dogs or cats as long as they are not in heat
6. Communication
   - We use "Whatsapp" to communicate with owners
   - We send pictures and videos time to time to update your pets
7. Social Media
   - We upload daily photos to Facebook and Instagram

8. Rate
   - $22 / day for a dog
   - $35 / day for two dogs
9. Payment Policy
   - After you submit this form, I will be sending you invoice by using quickbooks:
   - You can pay directly through Quickbooks before drop off,
   - OR you can pay with cash, credit card, etc. in person (square).
   - OR you can e-transfer full amount in the morning on the drop off date to
        maplelatte_home@outlook.com

    - GST will be added on all transaction
11. Prepayment Policy for Doggy Daycare
    To ensure a smooth and efficient experience, we require prepayment for daycare sessions in the following cases:
    - Single-Day Visits: No prepayment is required; payment can be made on the day of service.
    - Multi-Session Packages: Clients can purchase prepaid packages of 5, 10, or 15 sessions. These sessions will be tracked and can be used at any time.
    - Please notify us at least 48hrs prior.
    
    Expiration & Refunds: Prepaid sessions are non-refundable and must be used within [a year] from the date of purchase.
    
    
    Please ensure you are certain before making the payment. However, if you choose to pay the full amount before the service begins, please note that all payments are strictly non-refundable under any circumstances

We open daily: 0700hrs to 1700hrs
Also, open on holiday: 0800hrs to 1700hrs

When you book with us, please provide drop-off time.
Latest pick-up time would be 1800hrs.

I, the undersigned pet owner, hereby acknowledge and agree to the following terms:

1. Assumption of Risk: I understand that while [Maplelatte Home] strives to provide a safe and comfortable environment for my pet, there are inherent risks associated with boarding animals. I assume all risks related to my pet’s stay.
   
   
2. Veterinary Care Authorization: In the event of an emergency or illness, I authorize [Maplelatte Home] to obtain veterinary care for my pet at my expense. I agree to pay all veterinary costs incurred and understand that efforts will be made to contact me prior to treatment, if possible.
   
   
3. Health and Vaccination Requirements: I certify that my pet is in good health and has received all required vaccinations. I will provide proof of vaccinations (Rabies, Distemper, Bordetella, etc.) prior to boarding. I understand that pets without proof of vaccinations may be refused.
   
   
4. Behavioral Assessment: I acknowledge that [Maplelatte Home] will assess my pet’s behavior upon arrival. I understand that any aggressive or dangerous behavior may result in my pet being removed from the facility immediately, and I will be responsible for any associated costs.
   
   
5. Release of Liability: I hereby release and hold harmless [Maplelatte Home], its owners, employees, and agents from any and all liability for injury, loss, or damage to my pet or my property while my pet is in their care, except in cases of gross negligence.
   
   
6. Pet Care Instructions: I have provided [Maplelatte Home] with any specific care instructions for my pet, including feeding, medication, and behavior notes.
   
   
7. Photography Release: I consent to [Maplelatte Home] using photographs or videos of my pet for marketing or promotional purposes, unless I provide written notice otherwise.
   
   
8. Emergency Situations: I understand that in the event of an emergency that requires immediate action, [Maplelatte Home] may make decisions regarding the care and treatment of my pet, and I will not hold them liable for such decisions.
