INSTRUCTIONS

PLEASE READ BEFORE PROCEEDING

Please complete a separate form for each meet that should be included on the 2026-27 Fall/Winter Wisconsin Swimming Meet Schedule (aka Short Course Season).

The meet details entered thru this form will be the only information reflected on the schedule. Please be sure to check your details for any omissions before submitting.

SAVING YOUR WORK
If you need to stop in the middle of completing the form and leave the site, you can come back to where you left off by clicking the SAVE button. A window will then pop up. Click SKIP Create Account at the very bottom to get the link to your partially completed form. When you click on the link from the SAVE option, you go back to your form. Click on REVIEW ANSWERS.

AFTER SUBMITTING THE MEET TO THE SCHEDULE
If you have more than (1) meet to submit, you will have the option to return to the form after each submission.

Be sure to check your info before clicking the Green SUBMIT button.
- Have a representative attend the House of Delegates Meeting on April 25, 2026 to answer any questions about the meet.
- It is helpful to include any additional per athlete SURCHARGES. These are fees added to the individual entries: Admissions, Facility Fees, parking, heat sheets, etc.
  If you are collecting admission at the door, do not list it as a surcharge.
  DO NOT INCLUDE THE WI SWIMMING SPLASH FEE
- You will need the Meet Director's USAS Member ID # to complete the schedule submission
- A copy of the info in this form will be emailed to the Contact Email listed in the first section. The email will be your record for bid submission. There is not a PDF of this form available at this time.
