Oregon’s 2022-2027 Medicaid 1115 Demonstration Waiver includes expanded benefits for some Oregon Health Plan (OHP) members. Those facing challenging situations may be eligible for housing, climate, and nutrition support. OCFSN convenes a statewide working group of Veggie Rx programs and other nutrition and food access operators who are interested in becoming providers of these health-related social need (HRSN) benefits. OCFSN has received Community Capacity Building Funding (CCBF) from EOCCO and Trillium to support convening, facilitation, training, and support to manage all aspects of the Health Related Social Needs (HRSN) Food Provider Statewide Learning Collaborative.
To help better serve all Oregonians access healthy foods, OCFSN is looking to contract for services to deliver on the Scope of Work outlined in Attachment A. The selected service provider will report to the OCFSN Executive Director of Network Development and work closely with the OCFSN Veggie Rx Steering Committee.
This RFP outlines the timeline, Scope of Work, deliverables, budget, and how to submit a proposal for this contract. Introductions to the Veggie Rx working group and onboarding to OCFSN systems (Zoom, Google Programs, etc) used to implement the program will be provided.

The contractor will convene Community Based Organizations (CBOs) who are interested in providing Medicaid transition population members with food and nutrition benefits and want to be part of OCFSN’s Health Related Social Needs (HRSN) Food Provider Statewide Learning Collaborative (Learning Collaborative). The contractor will facilitate peer learning and technical assistance specifically for food-related program operators. The purpose of the Learning Collaborative is to increase the capacity of CBO partners so that they may be adequately prepared to provide HRSN benefits. The contractor will need excellent communication skills, a familiarity with Food as Medicine models, and an understanding of Medicaid service delivery and HRSN benefits in Oregon.
The contractor will be responsible for implementing the Scope of Work identified in Attachment A.
Timing, Hours and Budget:
OCFSN has a not-to-exceed budget of $20,216 for this scope of work. The contractor reports to the OCFSN Director of Network Development and works closely with the Veggie Rx Steering Committee.

Independent Contractor Status. It is understood that the Contractor, while performing services pursuant to this Agreement, will at all times be acting as an independent contractor and not an employee of OCFSN. The manner and means of providing the required services are under the sole control of the Contractor. The Contractor shall supply all necessary tools, materials and supplies for the performance of the services required.
Proposals will be accepted through November 22nd, 2024. The Review Committee will review on a rolling basis and make an offer to the selected candidate.
OCFSN will prioritize proposals received from qualified OCFSN member organizations or affiliated food systems organizations in Oregon, but will consider proposals from all qualified individuals operating under any legal business structure, including sole proprietors.
