1.) CANCELLATIONS
(a) In the event that a member decides they no longer want to continue with the classes, any remaining paid classes cannot be converted or transferred for use as general gym membership or access to other services. All unused classes will be forfeited upon cancellation, and no refunds will be provided for these classes.
(b) If a member does not show up for a class, the member will not be refunded for that class nor will that class be moved forward.
(d) In the event that the trainer is unavailable, another trainer will be assigned to conduct the session. If no other trainer is available at the scheduled time, the class will be carried forward.
2.) TRANSFER
The member is not allowed to transfer any pending sessions to another individual or another service provided by the gym.
3.) MEMBERSHIP FREEZE
The member acknowledges and agrees that this membership is non-freezable for any reason. This includes, but is not limited to, personal circumstances, travel, medical conditions, or other unforeseen events. The membership term must continue uninterrupted from the start date until the end date as specified. No requests for freezing the membership will be accepted under any circumstances.
4.) LATE ARRIVALS
Sessions will not be extended due to the lateness of the member(s) or due to interruptions caused by the member(s).
5.) LIABILITY
In recognition of the possible dangers connected with any physical activity, the client hereby knowingly and voluntarily waives any right of cause of action of any kind whatosever arising from any liability which may or could accrue to the gym and its trainers.
6.) ALL THE INFORMATION I HAVE GIVEN IS CORRECT
All the information on this form is correct and to the best of my knowledge. I have sought and followed any necessary medical advice. I understand that all the information given will be kept confidential.
