Basketball 3-on-3 Tournament
Welcome to the St. Rose High School 3v3 Basketball Tournament Registration Form!

We are excited to host this event on November 23rd for boys and girls in grades 3rd-8th. Each team will compete for awards, with winners announced in each grade division. Teams are guaranteed to play at least three games, and you can either make or bring your uniforms.
- Team Registration: You can register a team of 3-4 players (no more, no less).
- Individual Registration: If you are registering as an individual or with one other player, we will do our best to place you on a team that matches your grade level and skill level.
- Cost: The registration fee is $25 per player or $100 per team.
This form should be completed by the responsible party for the team. You will be asked to provide your contact information and the contact details for each player’s primary parent or guardian. Later, parents will receive a liability waiver via email to complete. All event communications will be directed to the responsible person completing this form.

To help you plan your day, here are our tip off times:

9AM: Grades 3 & 7
10AM: Grade 4
11AM: Grades 5 & 6
