Vaccine Availabilty Notification

By filling out this form you will be given the option to have a notification sent to you when we receive vaccines that you are looking for, and we will then be opening up new appointments.  

You will be asked how you wish to be notified, we will do our best to let everyone be notified at the same time.  It will be up to you to make an appointment as soon as you are notified.  

Being notified is not a guarantee that you will get a appointment, but you will know that we have just received vaccines and are opening appointments.  

You can still check for open appointment dates on our webpage, but use of this form will reduce you having to check for open appointments multiple times per day.

You will now pick which vaccine you wish to be notified when we have received more.  Other vaccines such as Tetanus/Whooping Cough, RSV Pneumonia, Regular Pneumonia, Shingles(Shingrix) are generally carried all the time, and you can check our website for availability and book an appointment through our website.

 

You will see a large green checkmark at the end of this questionnaire by pressing submit indicating you have successfully entered your choice.

Remember, once you receive notice of more vaccination appointments opening, be sure to go to our webpage, www.lopezislandpharmacy.com to make an appointment, you are only being given notice that we have vaccine and not being guaranteed an appointment.
