Scholarship Payout Procedures and Request Form

Policies and Procedures
- Scholarship awards can be used for a 2-4 year college or university, trade school or certification program.
- Scholarship funds are not paid out until the recipient is enrolled at a college/university, trade school or certification program and has proof of tuition paid or owed.
How to request scholarship funds:
1. Fill out the below form and submit.
2. Send proof of tuition or fees in the form of an invoice or receipt from the school or certification program you or your student is attending to the ISBD.
3. Once we've received the form and the proof of tuition, the ISBD will submit the request to the bank.
4. The scholarship check will then be sent directly to the school or certification program.
5. To submit proof of tuition, you can email a PDF to our office to soapbox@soapboxderby.org. Please use the subject line "Scholarship Proof of Tuition." Or, you can send a copy by mail to the office at:
