I, the undersigned, hereby grant Bethel Empowerment Church Las Vegas permission to use my written and/or video testimony. I understand that the video submitted/recorded may be used in church publications mentions, on the church website, social media platforms, and other promotional materials for the purpose of celebrating and promoting the church’s activities. I authorize Bethel Empowerment Church to edit, alter, copy, or distribute the videos or photos for social media advertising and marketing.

I further acknowledge that I will not receive any compensation for the use of my written and/or video testimony and that all rights will belong to Bethel Empowerment Church Las Vegas.
