This Childcare Agreement ("Agreement") is made between Dancy Daycare ("Provider") and Parent/Guardian listed above. ("Parent/Guardian"), regarding the care of Child listed above. (child)
1. Services Provided
Dancy Daycare agrees to provide childcare services for the Child in a loving, safe, and nurturing environment. Dancy Daycare specializes in incorporating biblical principles and wholesome values into daily activities, along with creative movement and dancing, designed to foster spiritual, physical, and social development.
2. Ages Accepted
Dancy Daycare will accept children between the ages of 2-6.
3. Fees and Payment Schedule
Enrollment Fee: $135 (first week one-time, non-refundable)
Weekly Tuition Fee: $135.00, due each Friday for the upcoming week’s care.
Late Payment Fee: $10.00 will be charged if payment is not received a week late.Daily Tuition is $55.00 a day, due at drop off .
Fees are subject to change with a 30-day notice provided in writing to the Parent/Guardian.
5. Activities
The following activities will be included in the daily routine at Dancy Daycare:Biblical Teachings: Short devotionals and age-appropriate Bible stories that instill biblical principles.
Creative Dance and Movement: Daily dance activities designed to improve coordination and self-expression.
Arts and Crafts: Projects to foster creativity.
Outdoor Play: Supervised playtime in the yard for physical development.
6. Meals and Snacks
Dancy Daycare will provide nutritious meals and snacks daily. Parent/Guardian is required to inform the Provider of any allergies or dietary restrictions.
7. Health and Safety
Parents/Guardians must provide up-to-date immunization records for the Child.
Children must not attend Dancy Daycare if they are ill, contagious, or show symptoms of a contagious illness.
If the Child becomes ill during care, the Parent/Guardian will be contacted immediately to arrange for prompt pick-up.
8. Pick-Up Policy
Only authorized individuals listed on the Child’s Emergency Contact Form may pick up the Child from Dancy Daycare.
A valid photo ID will be required for pick-up.
A late pick-up fee of $7.00 will be charged for each hour after 5:00pm.
9. Vacation and Holidays
Dancy Daycare will be closed on the following holidays: Christmas eve/Day, Thanksgiving eve/day, News years eve/day. You will be inform 2 weeks advance when owner is going on a vacation.
A minimum of two weeks' notice is required if the Parent/Guardian intends to withdraw the Child or pause childcare services for an extended period.
10. Termination
Either party may terminate this Agreement with two weeks' written notice. In the case of immediate termination due to non-payment or violation of Dancy Daycare policies, no notice is required.
11. Special Considerations
Dancy Daycare agrees to communicate regularly with the Parent/Guardian regarding the Child’s progress and any concerns.
Parent/Guardian are responisble to provide necessary supplies (diapers, extra clothes, etc.) as discussed with the Provider.
12. Emergency Medical Authorization
In the event of an emergency, Dancy Daycare is authorized to seek medical care for the Child if the Parent/Guardian cannot be reached. Parent/Guardian agrees to bear any costs related to such medical care.
You must complete an Authorization to Administer Medication Form for all prescriptive and non-prescriptive medications that need to be administered at childcare.
This contract represents the full understanding between Dancy Daycare and the Parent/Guardian concerning the Child's care. Any modifications to this Agreement must be made in writing and signed by both parties.Should you have any questions or concerns, please notify me before signing this contract. By signing, you hereby acknowledge that you have entered into a legally binding contract. You also acknowledge that you have received and agree to abide by the policies and procedures outlined. I may amend the contract by giving you a copy of the new or changed policies at least one month before any changes go into effect.
