  • If you have any history of diabetes, hypertension or any other situation that effects kidney function, labs are mandatory prior to initiating treatment.  If you have had labs done in the last 6 months you may upload them below.

These can be ordred at any lab (if you would like to use your medical coverage insurance) or can be done at Creekside North (if this option is more affordable than co-pay/co-insurance/deductible).  I have NO way of knowing which labs your insurance covers or to what extent.  Please contact your insurance directly.

Factors, such as hormone imbalances, thyroid dysfunction, insulin resistance, or nutrient deficiencies, can contribute to difficulty losing weight. The following list is not all-inclusive but represents some of the more common reasons why weight loss may be challenging.

If you have never had these tested or are experiencing symptoms like fatigue, cravings, or stubborn weight gain, you may want to consider checking your levels. If you are unsure, we can discuss whether additional testing would be beneficial during your consultation, and we can always add labs afterward if needed.

If you would like any additional lab testing at this time you will have the option to have them done at Creekside North, Take a lab order to the lab of your choice, or we can fax an order.

For LABS only, we DO take insurance.  Our labs are drawn in office, and processed through QUEST labs.  There is a $35 lab draw fee, which is less than most labs charge.  THIS IS SEPARATE FROM ANY BILL YOU WILL RECEIVE FROM QUEST OR YOUR INSURANCE.
