• Daily Shift Log

    To be completed by Shift Officer for each 24 hour period. (rev: 2/2/25)
  • Purpose: This log is to be used to document shift activities, incidents, equipment status, etc.

    Each submission should capture the previous 24 hour period.

    This form can be worked on during the shift and saved. Only hit "Submit" once you are deeming it an official log entry.

    Once submitted, this is considered final and is an official record of the shift. If, after submission, a correction is required, please contact the Fire Chief.

    If you mistakenly hit "Submit" for multiple forms, please contact the Fire Chief to identify which one should be recorded "Official".

  • ALL RESPONSES should have at least a quick debrief, if not an informal after-action review.

    Identify and reinforce all positive actions/decisions.

    Together, shift should identify, discuss, and resolve any areas needing improvement.

    If you feel that a more formal AAR is needed, please indicate as such and provide brief discussion on your thinking.

  • Crew Health (NEW 2/2/25)

    As we've seen, keeping an eye on each other is crucial and important. While we all should be looking out for everyone, as an Officer/Shift Lead, it is an added duty that should not be taken lightly.

    The question below is a simple "Yes/No" answer, and only included in order to ask you to think about it.

    If you answer "Yes", I will reach out to you as soon as I'm able.

    If you feel you do need to discuss anything, but would rather not "disclose" it here, simply answer "No", and then reach out to me directly. No matter what the answer is each day, it in no way locks any of us down to that answer.

    Of course, I am always available - if there is an immediate need, DO NOT WAIT TO USE THIS FORM TO CONTACT ME. Contact me directly as soon as you are able.

  • Missing and Pending Reports (NEW 2/8/25)

     

    If not all reports have been submitted or if there are reports that are pending ("Needs Crew Attention", "MD Comments", etc.), list each report, who is assigned, explain why not completed that shift, and when it will be completed. For example:

     

    -2024-0042 (EMS, Winiarski) - missed that it requires crew attention, he will complete by end of day 2 of this shift.

    You can work on and save this log through the shift. To save, hit the "Save" button and you should get an email with a link to come back to the form.

     

    FINAL SUBMISSION

    Once complete, hit the "Submit" button. By submitting, you are signifying that the log is complete and accurate.

    If, after submission, you need to make any corrections, contact the Fire Chief.

  • Should be Empty:
