I understand that by submitting this form, the student is requesting admission and registration in the selected program of study. I understand that I will be admitted to the Enzweiler Building Institute (“EBI”) for the 2025-26 school year without further correspondence from the school.*

I understand that a $500 deposit is due at the time of application in order to secure my spot in the 2025-26 after-school program.*

I understand ALL CANCELLATIONS MUST BE IN WRITING. ONLY A WRITTEN LETTER OR EMAIL TO THE DIRECTOR OF WORKFORCE DEVELOPMENT IS AN ACCEPTABLE FORM OF CANCELLATION. No other form of cancellation will be accepted. Cancellations are only accepted when received prior to Septermber 2, 2025 (the first day of the academic year).*

I understand that I will receive a 100% refund of all tuition paid if I cancel in writing after receipt of my application but prior to the first day of the academic year (September 2, 2025).*

I understand that if I elect to discontinue (withdraw from) the training program in writing between September 2 and October 2, the program will calculate a daily tuition rate for days attended, and I will receive a refund for any remaining tuition, less a $150 termination fee.*

I understand that if I elect to discontinue (withdraw from) the training program in writing after October 2, I will not receive any refund of any kind and will be assessed full tuition and fees for the academic year.*

I understand that classes and curriculum are subject to change without notice at the discretion of the EBI. Curriculum or instructor changes will not result in a refund.*

I understand it is my responsibility to follow all classroom procedures, rules and expectations, especially as relate to my safety in the program. I understand my repeated failure to follow all classroom procedures, rules and expectations may result in my suspension or dismissal from the program.*

I further understand the process for addressing complaints is detailed in the EBI Student Handbook. I have the right and responsibility to follow that process should I have a complaint.*
