I understand that by submitting this form, the student is requesting admission and registration in the selected program of study. I understand that I will be admitted to the Enzweiler Building Institute (“EBI”) for the 2025 school year without further correspondence from the school.*

I understand ALL CANCELLATIONS MUST BE IN WRITING. ONLY A WRITTEN LETTER OR EMAIL TO THE DIRECTOR OF WORKFORCE DEVELOPMENT IS AN ACCEPTABLE FORM OF CANCELLATION. No other form of cancellation will be accepted. Cancellations are only accepted when received prior to January 6, 2025 (the first day of the academic year).*

I understand that I will receive a 100% refund of all tuition paid if I cancel in writing after receipt of my application but prior to the first day of the academic year (January 6, 2025).*

I understand that if I elect to discontinue (withdraw from) the training program in writing between January 6 and February 7, the program will calculate a daily tuition rate for days attended, and I will receive a refund for any remaining tuition, less a $150 termination fee.*

I understand that if I elect to discontinue (withdraw from) the training program in writing after February 7, I will not receive any refund of any kind and will be assessed full tuition and fees for the academic year.*
