    • The Gage County Achievement Awards and Scholarship Application consists of three parts:

      • 4-H Project Accomplishments
      • Leadership & Volunteer Experiences
      • 4-H Project Awards, Special Awards & Scholarship Selection

      Instructions:

      • There are two ways to fill out the Gage County 4-H Achievement Awards & Scholarship Application:
        1. Click on the link and fill out the information on the link and click "submit" when finished. OR
        2. Download the .pdf to your computer. Type your answers in each field on the downloaded .pdf. Make changes to the .pdf and save regularly.
          • When you are finished, click SUBMIT at the end of the form. Your answers will be saved and sent to the Extension Office.
          • You will recieve an email indicating that your submission has been complete.
            Feel free to save the .pdf to your computer for your own records.
    • Applicant Information

      Include the following general information about the award/scholarship applicant.

    • Please indicate up to two clubs that you're applying under. Clubs will recieve points for applications that are submitted.

    • Project Accomplishments

    • In this section, list the projects that you worked on this year. Describe your project work and what you learned. It is not a requirement that you complete a county fair exhibit to list the project. List any workshops/camps/learning experiences that you attended relating to the project area.

      Example:

      Clothing Project - Attended a sewing workshop with my club, made pants to exhibit at the county fair, pants received a purple ribbon and were selected for the state fair, received a blue ribbon at the state fair.

      Woodworking Project - Learned to use a router.

      Bucket Calf Project - Attended livestock judging clinic, purchased and fed a bucket calf, exhibited calf at the county fair, and entered in the livestock judging contest at the county fair.

    • Leadership & Volunteer Experiences

    • In this section, list any 4-H or non-4-H leadership, volunteer, or community service experiences that you did in the past year. Describe what you did.

      Example:

      4-H Club Secretary - Recorded minutes at the 6 4-H club meetings I attended.

      4-H Workshop Helper - I helped with the 4-H Bread in a Bag Workshop held at the Extension Office.

      Hygiene Closet - I purchased items for the hygiene closet and delivered them to the Middle School.

      Band - I played second-chair trumpet.

      Basketball - I was on my school basketball team.

    • Part 1: Project Area Awards

    • Instructions:

      You can apply for up to 3 County 4-H Project Awards.

      1. Use the dropdown menu to select the County 4-H Project Area of the award you want to apply for. Not sure what an award includes? Check out this listing.
      2. For each of the awards you are applying for, pick an experience you would consider a “highlight” of your year.
        1. In 2 paragraphs or less, describe this experience and how you have grown from when you started the project to now.
        2. Answer any of these questions that apply: What did you do? What was involved? Who was a part of this experience? Why did you want to participate? What was accomplished through this experience? What was your favorite part of the experience? What did you learn? 

      **Note: If you do not wish to apply for 4-H Project Area Awards, skip this section.

    • Part 2: Special Awards

      If you do not wish to apply for Special Awards, skip this section.

    • You can apply for any (or all) of the special awards:

      • 4-H Rookie Award - you can be a new 4-H member or new to a project area
      • Memorable 4-H Experience
      • 4-H Hard Luck Award

      Instructions:

      For each award that you are applying for, write a short story (3 paragraphs or less) describing why you should receive the award. Describe what you did and learned.

      If you do not wish to apply for a Special Award, please leave the box empty.

    • Part 3: Scholarships

      If you don't want to apply for Scholarships, please continue to the end of the form.

    • Your application will automatically be considered for the 4-H Scholarships, including:

      • Ward & Hazel Finch Scholarships (2 available, $50 each)
      • John Rhodes/Beatrice Kiwanis Club Scholarship (1 available, $50)
      • C.A. Anderson Memorial Scholarship (1 available, $50)
      • Odell Cookies 4-H Scholarship (1 available, $25)
      • Gage County 4-H Council Scholarships ($50) – 2 livestock, 1 dairy, 2 horse, 3 general.

      In this section, you will indicate if you want to apply for a special scholarship, including:

      • “Sewing Seeds for Tomorrow” (1 available, $100) sponsored by Gary & Mary Jo Bures - Ages 12-15 (focus on Clothing & Horticulture/Floriculture project areas)
      • Neva Krauter Memorial Scholarship (1 available, $25) - To be used for the Citizenship Washington Focus Trip.
      • “Excellence in 4-H” Award (2 scholarships per age division, $50) sponsored by Exmark
        • Jr. Division - Ages 12-14
        • Sr. Division - Ages 15-18
      • “Kids of Character Award” (2 available, $100) in memory of Gwendolyn Cooper - Ages 9-12
