Event Pet Waiver Form

1. I confirm that my dog is up to date on all required vaccinations, including rabies, distemper, and parvo.


2. I verify that my dog has not exhibited any signs of illness in the past 7 days, such as coughing, runny nose, fever, lethargy, eye discharge, or reduced appetite.


3. I agree to keep my dog on a leash at all times during the event, except in designated off-leash areas. Retractable leashes are not permitted.


4. I will clean up after my dog in the event of any messes or accidents.

5. I confirm that my dog complies with all municipal by-laws.


6. I ensure that my dog is well-socialized and comfortable in group settings. If my dog shows signs of stress, fear, or aggression, I will remove them from the event immediately.


7. I take full responsibility for my dog’s behaviour and will maintain control of them at all times while attending Barkade Social events.


8. My dog is not aggressive, not in heat, and has no history of biting or showing aggression. My dog has not caused any property or bodily damage.


9. I agree to abide by all event rules and policies. Failure to do so may result in being asked to leave the event.


10. I understand that Barkade Social and its affiliates are not liable for any injuries, damages, or losses incurred during the event. I assume full responsibility for my dog’s actions and any consequences thereof.


11. I grant Barkade Social permission to use any photographs or videos taken of me and my dog during the event for promotional purposes.


12. I certify that all information provided in this registration and waiver is accurate and complete. 
 


I acknowledge that by attending a Barkade Social event with my dog, there are inherent risks involved, including interactions with other attendees and their dogs. In consideration of being allowed to participate, I voluntarily accept full responsibility for any risks to myself, my dog, or other dogs.

I hereby waive and release any and all claims against Barkade Social, its organizers, event venues, employees, volunteers, and sponsors from any liabilities, damages, expenses, or fees that may arise from or relate to my attendance at the event. I agree not to pursue legal action for any such losses.
